Profile Updates

This form is currently compatible with Firefox, Chrome and Safari. This form will not load with Internet Explorer.

You can submit a request to create or edit a Boston Children’s Hospital clinician profile by completing the appropriate form below.  The form will automatically generate a Remedy Ticket and forward it to the Digital Marketing Department.

• We are no longer doing online appointment scheduling for headshots. Instead, a self-service Iris Booth headshot photobooth is available in the main hospital’s badging office for everyone that meets the above headshot criteria. Click here for more information about headshots.
• Need to make a change or edit to your Publications? Log into your Harvard Catalyst account and update accordingly. Once updated, edits/changes should then be reflective to your profile.  All publications are pulled directly from Harvard Catalyst.
